Responsibilities & Duties
- Deliver skilled nursing care as outlined in each client’s care plan, under the direction of physicians and registered nurses.
- Monitor and assess clients’ health conditions, including vital signs, and communicate changes to the care team.
- Administer medications following approved protocols, including oral, topical, subcutaneous, and intramuscular routes.
- Assist clients with activities of daily living (ADLs), personal care, and other support as needed.
- Accurately document all client interactions and maintain thorough records to support continuity of care.

Requirements
- Current Licensed Practical Nurse (LPN) license in good standing with the regulatory authority in Ontario.
- Minimum of one year of nursing experience preferred; experience in community or home health care is an asset.
- Proficiency in English for both verbal and written communication (additional languages are an asset).
- Current TB screening and a clear criminal background check.
- Valid driver’s license and legal eligibility to work in Canada.
