Responsibilities & Duties
- Conduct thorough initial and ongoing assessments to understand each client’s unique health needs.
- Develop and implement individualized care plans in collaboration with the client’s healthcare team.
- Provide skilled nursing care, including medication administration, wound care, and catheter management, with precision and compassion.
- Monitor and document changes in client health and update care plans accordingly.
- Offer guidance to LPNs and Personal Support Workers, ensuring consistent, high-quality care.
- Accurately document client interactions and care provided, following regulatory standards and AvantiCare’s protocols.

Requirements
- Current Registered Nurse (RN) license in good standing with the College of Nurses of Ontario (CNO).
- Minimum of one to two years of nursing experience, preferably in home care or a community health setting.
- Proficiency in English for both verbal and written communication (additional languages are an asset).
- Current TB screening and clear criminal background check.
- Valid driver’s license and legal eligibility to work in Canada.
